You know I often see this post and it's completely wrong. So much stuff can happen at the beginning of fallout 3 before you leave the vault. You have a choice on how you treat Amata. Whether or not you choose to kill the overseer. Whether or not you collect the bobblehead.
Whether or not you killed Butch and/or saved his mother. The things that I have noted come back later on in the game when you revisit the vault and how the characters inside will perceive you even in fact which characters you can have and the state of the vault when you enter.
When you start a new game by loading a save at the end of the vault you force your character to have the same back story as the first time you have done it. This negates all the choices that you could of made.Don't even get me started on all the things you can do to/for Good Springs before you get that save scene in New Vegas.
